Crystal Awards are presented each year to honor individuals who have demonstrated exemplary service and have made contributions to the betterment of the Sun Lakes Community. Anyone may nominate a worthy community volunteer.

Many of us know several who go above and beyond in many ways to serve our HOA, service groups, religious groups, and various other organizations and clubs that make Sun Lakes a wonderful place to live. So, show your appreciation for that volunteer by honoring him or her by submitting a nomination.

Nomination forms are available at each one of the HOA administrative offices. Also, for your convenience, you may complete the form online and then download it. Just click the button below. The deadline for submission is December 8th at 4:00pm. You may submit your nomination form at your HOA administrative office or mail it to PO Box 12402, Chandler, AZ 85248.

There will be a dinner gala at Oakwood Ballroom on March 22nd for the honorees and their friends and family.
